Need a notary for the student participation form? There will be one available, parent and driver license, must be present.

Parents create an account at athleticclearance.com for your athlete, be sure to include all sports your athlete wants to join.
Helpful tips: Student must make accounts on NFHSlearn.org and add 3 courses to their cart and checkout (no cost). See screenshots!
Once they are complete, upload the certificates to website here.
The notarized form was missing at the beginning, and it was just added, I am attaching it here if you need it and can’t find it on the website. Complete form and get notarized and upload to the Athletic Participation Form on website. Then upload it here on the Athletic Clearance website.
